Texas Hospitals are Leading Initiatives to Reduce the Number of Re-hospitalized Patients

(Austin  –  March 3, 2014)  –  Texas hospitals are keeping patients healthier and reducing costs by implementing strategies to keep discharged patients from being re-hospitalized because of a preventable complication or other adverse event. From preventing infection and simplifying discharge instructions to engaging families and outpatient providers, hospitals are involved in multiple initiatives with the single goal of making sure patients do not have to return to hospitals unnecessarily.

Hospitals are making these investments in quality and patient safety despite having a persistently high number of uninsured patients and fewer resources because of major state and federal funding cuts.

“Texas hospitals are leading the way on reducing re-hospitalizations because of their commitment to providing the right care at the right time for every patient,” said Ted Shaw THA president/chief executive officer. “Regardless of their size, location or ownership status, they are all investing in technology, education and personnel to improve patient safety and quality of care and to reduce avoidable re-hospitalizations.”

Why some patients are re-hospitalized depends on a number of complex and interrelated factors. For this reason, Texas hospitals are involved in multiple strategies to address the problem, including:

  • Utilizing technology innovation such as predictive analytics to identify patients at high potential risk for a readmission.
  • Providing better and safer care during the initial inpatient stay, such as reducing the risk of infection.
  • Paying the strictest attention to patient medications.
  • Employing case managers to support discharged patients.
  • Improving communication with patients and caregivers regarding follow-up care and discharge planning.
  • Improving care transitions through better communication and collaboration with community providers.
  • Adopting best practices through national quality and patient safety initiatives, such as the Centers for Medicare & Medicaid Services’ Partnership for Patients and collaborating with community providers through initiatives such as CMS’ Community Care Transitions Program.
